What is it? Child Health Plus is a New York State health insurance program for children under age 19. If your children are not eligible for Medicaid, they may be eligible for this program. What do you get? Child Health Plus offers many health services for children including physical exams, immunizations, emergency treatment, dental and vision care, and hospitalization. These health services are provided through a managed care plan. A managed care plan is a health plan provided by a private Health Maintenance Organization (HMO). You can choose from a number of managed care plans. How long do you get it? You must re-certify once a year in order for your children to continue receiving Child Health Plus. Your Child Health Plus enroller will send you a re-certification application. |
